Customer Service Survey - Advice Needed

I'm on a brand-new committee to create and distribute a survey to our customers regarding our service. We already know the first wave will be sent out via email (developers have to submit plats online, so we have a built-in database of accounts). This is supposed to cover topics such as:

    wait times
    accuracy of information
    communication
    training
available materials (web page, paper, etc)
    ordinance interpretation

Has anyone done something like this before? Any advice, examples, warnings would be greatly appreciated.
"We do not need any other Tutankhamun's tomb with all its treasures. We need context. We need understanding. We need knowledge of historical events to tie them together. We don't know much. Of course we know a lot, but it is context that's missing, not treasures." - Werner Herzog, in Archaeology, March/April 2011

The closest thing I've done is a general customer service/perception survey for all city services and budget priorities. It doesn't get as specific as it sounds like you are going after. If you want, I can send it as an email later on (if I can find the file ).

"Oh, that is all well and good, but, voice or no voice, the people can always be brought to the bidding of the leaders. That is easy. All you have to do is tell them they are being attacked and denounce the pacifists for lack of patriotism and exposing the country to danger. It works the same way in any country."

That would be great. If you find the file, just send it over. Thanks!
