    Certification form

    Does anyone have a copy of a certification of posting, mailing, etc. that they could post or email to me? I need to certify that I mailed out notices to multiple addresses.

    Here's what we use:


    STATE OF ________ ) AFFIDAVIT OF MAILING
    ) ss. [Case No.]
    City of _________ ) [Name of matter]


    I, [name] being duly charged do certify that on [date] I mailed a Notice of __________________________ to all property owners within 100 of the exterior boundary of the property, which parties are listed in the attached Exhibit A, for a ____________________ permit which is herein incorporated by this reference.

    I further certify that the attached Exhibit B is a true and accurate copy of the notice, which is incorporated by this reference.
