    That's the Boston Avenue Methodist Church in Tulsa, OK.

    That's one of the premier art deco churches still in existence IMHO
    People will miss that it once meant something to be Southern or Midwestern. It doesn't mean much now, except for the climate.     Maister,

    You nailed the place, but I am afraid I can not award you any bonus points.

    The architect I was looking for was Bruce Goff, I would have also accepted Adah Robinson. There is some dispute on the involvement of both architects, but I have most often heard it attributed to Goff.

    For confirmation of the architect, refer to the wikipedia article you cited in you post...

    I do agree that it one of the premier examples of religious art deco architecture.
