If you're up for a drive then Zingerman's in Ann Arbor is probably worth the pilgrimage (but stretches the 'local' part for me). Is pizza your thing? Chicago Style Pizza is the best I've had outside of Chi town.

Regarding Zingerman's.... everything I've ever had there is the Best of its category, but I'd consider it a stretch as far as local goes as well. Sandwiches - Awesome! Breads, rolls, pastries - Awesome! Ice cream - Awesome! Desserts - Awesome!
Closer to me, if I want the best pizza, I go to Buddy's for authentic Detroit deep-dish square pizza. For a great Polish meal I head to the Polish American Cultural Center. When I was still in grad school, I'd head to Avalon Bakery in Detroit for the best breads and rolls (this side of Ann Arbor anyway).

Italian: Guercio's.
Horseradish: Famous Horseradish
Butter lambs: Malczewski's
Cheese: Big Cheese in Fort Erie, or Cuba Cheese Shop in Cuba.
Sponge candy: Alethea's
Sausage (generally): Spar's
Of these, the most pilgrimage-prone are the Cuba Cheese Shop, and "da Brudvaymarket's" during Easter.

Say it isn't so! I found that place in 1969 when I was stationed at Hunter Army Airfield in Savannah. According to the article it had been there since 1934. I haven't been there in years but it was a required stop for me anytime I was in the area.
