    Customer service comment card template?

    Hello! I haven't been on the forums in forever! Nearly a year ago I was promoted to a director position within the municipality I work for. We are working through some customer service improvements and I wondered if anyone has ever sought feedback from community development department walk-ins by way of a comment card. If so, any words of wisdom on how the program worked (or did not) and any templates you'd be willing to share?

    Just a thought, but how about using an online survey service? Collect the eamil address of applicants and send them a brief survey after their visit or after their project have been acted upon.
    Anyone want to adopt a dog?
