It seems like it'd be a good idea at any major intersection, not just round-a-bouts. Of course that'd be pretty cost prohibitive.

I don't know what's scarier.. newly built ped crossing bridges in Las Vegas or the fact that the google maps image actually show tons of people using 'em...
I was thinking full round style with access to all corners, not just the up and over.
Besides, they could put some slot machines on it and pay for themselves.
